Hawaii Medical Cannabis - 329 Cards 

Hawaii state Department of Health certified & regulated medical cannabis / medical marijuana program. 

Hawaii 329 cards provide streamlined access to a wide variety of cannabis through ​medical dispensaries. You can also grow medical cannabis at home with complete legal legitimacy and security of privacy.

Eligible Conditions:

  • ​Amyotrophic Lateral Sclerosis (ALS)

  • Cancer

  • Glaucoma

  • Systemic Lupus Erythematosus (SLE)

  • Crohn's disease

  • Epilepsy

  • Multiple Sclerosis (MS)

  • Rheumatoid Arthritis (RA)

  • HIV/AIDS

  • Post-Traumatic Stress Disorder (PTSD)

  • “The treatment of these conditions”, or “A chronic or debilitating disease or medical condition or its treatment that produces one or more of the following":

    • Cachexia or wasting syndrome

    • Severe pain

    • Severe nausea

    • Seizures, including those characteristic of epilepsy

    • Severe and persistent muscle spasms, including those characteristic of multiple sclerosis or Crohn's disease

Please note that certification by a medical provider does not guarantee approval for a 329 card if you do not meet the State of Hawaii's eligibility criteria. You must comply with all state regulations, including the prohibition against possessing a registered firearm. Additionally, a medical cannabis certification does not exempt you from employer drug testing policies.

Initial Certification

  1. Register for a Department of Health (DOH) account (one per person)- Click here to register

  2. Have your ID or Patient/Caregiver/Grow Site Certification Forms, as applicable ready to upload on the DOH website

  3. Designate your medical provider as Chelsea Rorer

  4. Pay the State Application fee of $38.50

 

  • A FACE-TO-FACE appointment is REQUIRED if you are a new client or were certified by another provider

  • Note: Mobile appointments may be available near the North Shore.

  • Be prepared to provide any information confirming your chronic condition.

30 Minutes

  • includes intake appointment and provider completion of online application

  • there is a separate state fee of $38.50
